Returning students: This year we are asking returning students to complete ALL the forms below before starting school. Thank you for your cooperation with this.

New students: Please complete all enrollment forms. Your child cannot begin school in September without a complete file.

All students: 

  • A current physical must be on file at Milldam.

  • Students with allergies or any health condition that could require medical attention at school - please reach out to Tara (director@milldamnurseryschool.org). She will work with you to fill out the “Individual Health Care Plan Form” and “Medication Consent Form.”

  • Review the Milldam Nursery School Manual linked at the bottom of this page and complete the sign-off form.
