Returning students: You only need to complete the Emergency Contact Form (no other forms) unless something has changed (e.g. health information/allergies or other medical conditions, transportation plans, authorized release persons, etc.).
New students: Please complete all enrollment forms. Your child cannot begin school in September without a complete file.
All students:
A current physical must be on file at Milldam. Please email it to the director.
Students with allergies or any health condition that could require medical attention at school need to fill out the “Individual Health Care Plan Form” and “Medication Consent Form” linked below and have them signed by their physician. Once completed please email them back to Karen.
Review the Milldam Nursery School Manual linked at the bottom of this page and complete the sign-off form.
